Four tiers of autonomy

Industry experts propose a four‑level framework. Level 1 is pure human care; Level 2 adds decision‑support tools that act like cruise control for clinicians. Level 3 hands entire diagnostic pathways to algorithms, and Level 4 envisions a fully autonomous system overseen by a handful of specialists.

“We are at the cusp of a shift that could democratize high‑quality care,” says a senior analyst at Bloomberg.

Recent FDA filings list over 1,500 approved AI/ML tools, 76% in radiology, most of which still require a human in the loop. The gap between Level 2 and Level 3 is narrowing as models surpass human scores on the USMLE, reaching accuracy above 90%.

Yet challenges remain. Data privacy, clinical validation, and reimbursement models must evolve. Without a clear roadmap, the technology could reinforce the very inequities dramatized in sci‑fi visions of space‑borne elites. The pandemic taught us that rapid deployment of digital health can save lives; now the sector must harness that momentum responsibly (pandemic).
