CTE prevalence in NFL players: Why the latest BMJ study may not spell doom for the sport

CTE prevalence in NFL players: what the new BMJ analysis reveals

In a study that examined 338 donated brains of former NFL athletes, researchers found that over ↓ 93% of the specimens displayed the pathological hallmarks of chronic traumatic encephalopathy (CTE). The work, published in the BMJ on Aug. 25, estimated that the overall CTE prevalence among the deceased cohort could range from ↑ 24.5% to ↑ 97.7% depending on how representative the donor pool was.

“The numbers are alarming, but they reflect a generation that played under very different safety rules,” says Thayne Munce, a concussion researcher at the University of Kansas.

Munce warns against extrapolating those figures to today’s athletes. Most of the men studied entered the league in the 1960s‑70s, a period when spearing was legal, helmets were rudimentary, and full‑contact practices were routine. Modern rule changes—penalizing head‑first hits, mandating helmet‑safety caps in practice, and limiting two‑a‑day sessions—have reshaped exposure.

Current protocols also prioritize rapid concussion identification and graduated return‑to‑play, a stark contrast to the “play through it” culture of the past. While the pathology of CTE can exist without overt dementia, the study noted that roughly 60% of brains with CTE signs also carried a dementia diagnosis, and the figure rose to 90% among stage IV cases.

For youth and high‑school programs, the risk calculus differs. Exposure to repetitive head impacts is orders of magnitude lower than in the professional arena, and equipment standards have tightened. Munce’s own research on adolescent players shows a measurable, yet modest, accumulation of sub‑concussive blows over a season.

What remains unknown is how today’s protective measures translate into long‑term brain health. As Reuters notes, CTE can only be confirmed post‑mortem, leaving a gap in real‑time diagnosis. Scientists are exploring blood biomarkers, advanced imaging, and even preventive pharmacology, but a definitive living test is still elusive.

Bottom line: the historic data underscore why the NFL has invested heavily in safety reforms, but they do not prove that current players face the same CTE prevalence in NFL players rates. Ongoing monitoring, smarter equipment, and continued rule refinement are the best bets for keeping the sport as safe as possible.
