Political ideology health gap widens as conservatives die faster

New research published in Nature reveals a stark political ideology health gap: by 2022, mortality among self‑identified very conservatives was ↓ 1.34% versus ↓ 0.2% for very liberals. The study tracked a nationally representative cohort from 2010 to 2022, controlling for age, income, geography and COVID‑19 deaths, yet the disparity persisted.

“In 2010 we could not detect a health divide; by 2020 the gap was unmistakable,” says co‑author Elizabeth Elder.

Trust in medical institutions emerged as the most plausible driver. Surveys show a sharp decline in confidence among conservatives, a trend that intensified during the pandemic and has spilled over to routine care such as blood‑pressure medication adherence.

Why mistrust matters

Older adults and those with chronic conditions are less likely to seek timely care, turning ideological skepticism into a mortality risk. The phenomenon aligns with the rise of anti‑vaccine activism and “Make America Healthy Again” rhetoric championed by figures like Robert F. Kennedy Jr., whose influence has helped erode public health messaging in several red states.
