Uterine Microbiome Breakthrough: Postbiotic Treatment May Extend Fertility in Older Women

How the uterine microbiome influences age‑related fertility

A recent study published in Cell Host & Microbe suggests that restoring a healthy uterine microbiome could mitigate age‑related decline in embryo implantation. Researchers examined endometrial samples from 149 Chinese women undergoing fertility treatment and found that older participants harbored fewer Lactobacillus species, notably L. gasseri and L. crispatus. In mouse models, daily vaginal administration of either live L. gasseri or its purified exopolysaccharide postbiotic (LG‑EPS) for ↑ 4 weeks reduced inflammatory markers and improved implantation rates. When the same regimen was applied to older female mice, embryo implantation rose to ↑ 56/93 compared with controls.

“The uterine microbiome appears to be a viable target for age‑related infertility interventions,” said Dr. Alex Robles, a fertility specialist at Columbia University.

The team isolated five Lactobacillus strains; L. gasseri showed the strongest adhesion to uterine epithelial cells, dampening oxidative stress and boosting production of exopolysaccharides. Removing the EPS‑synthesis gene stripped the bacterium of most benefits, pointing to the sugar matrix as the active agent. Human endometrial cells aged in vitro responded similarly to LG‑EPS, displaying enhanced receptivity to embryo attachment.
